Job Summary
Coordinate clinical, psycho-social, and spiritual services through the case management model while collaborating with the Interdisciplinary Team to meet patient needs. Conduct routine and emergency assessments, explain hospice services and Medicare benefits, and obtain informed consent documents. Perform supervisory duties for Hospice Aides and Licensed Practical/Vocational Nurses, adjust assignments as needed, and ensure comprehensive documentation at the bedside. Maintain updated care plans reflecting current problems, goals, and interventions, and communicate patient changes to the Medical Director and attending physician. Provide physical and emotional care during crises and prevent unwanted hospitalizations through anticipatory care in the home. Complete on-call services to provide holistic, team-based support for patients and families.
Required Qualifications
- Associate or Bachelor's degree in Nursing
- Active and unencumbered Registered Nurse license in state(s) of employment
- Current CPR certification
- Ability to lift and move objects up to 50 pounds
- Close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus
- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
- Ability to write reports, business correspondence, and procedure manuals
- Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties
- Strong written and verbal communications
- Ability to understand, read, write, and speak English
- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals
- Ability to compute rate, ratio, and percentage
- Regular standing, walking, and manual dexterity
