Job Summary
Inpatient UM Nurse Case Manager coordinates patient care: conducts comprehensive assessments, collaborates with healthcare providers to develop and implement care plans, monitors patient progress and adjusts plans as needed, facilitates communication among patients, families, and healthcare teams, ensures regulatory and accreditation compliance, educates patients and families about treatment options and resources, documents all case management activities per policy, and participates in quality improvement initiatives and team meetings.
Required Qualifications
- Active RN license in California
- Minimum 2 years nursing experience
- Experience in case management or utilization management preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work collaboratively in a fast-paced environment
- Knowledge of healthcare regulations and managed care principles
- Proficient in electronic health records and documentation practices
- Certification in Case Management (CCM) is a plus
