Job Summary
Provide clinical support, facilitate movement, and maintain a global view for hospitalized COPC patients while acting as a change agent for innovative healthcare approaches. Thoroughly triage complex patient issues, differentiate urgent needs, and consult with medical providers as required. Maintain knowledge of current processes, operations, and COPC outpatient resources to enable continuity of care and efficiency. Provide prompt communication with the hospital ED, consulting providers, and outside partners during patient admissions and tracking. Deliver a complete and comprehensive report to incoming RNs and providers while working effectively under physician supervision. Maintain confidentiality, document efficiently in the EHR, and react calmly in emergency situations.
Required Qualifications
- Current Ohio Registered Nurse (RN) License
- BSN preferred
- Minimum 3 years acute care nursing in a hospital setting
- Experience with EPIC electronic health record system
