Job Summary
Conduct comprehensive assessments of patient behavioral, psychological, and socio-economic needs upon consultation. Attend Care Planning or Discharge Planning rounds to facilitate communication regarding patient and family goals. Participate in end-of-life discussions and advance care planning while basing actions on the National Association of Social Workers code of ethics. Document measurement and evaluation of social service consultation outcomes as patients progress toward desired goals. Provide leadership to departmental staff and perform other duties as assigned.
Required Qualifications
- Masters Degree in Social Work or related field
- Certification, Registration & Licensure
- Minimum of three years clinical Social Work in a hospital, Skilled Nursing facility, Home Health or Hospice setting

- Must regularly lift, carry or push/pull less than 10 pounds
