Inpatient Social Worker Case Manager, Care Coordination (Full Time)

Inpatient Social Worker Case Manager responsible for coordinating and implementing hospital-based case management activities, serving as the patient/family/legal representative contact until transitions to another service line, reviewing care against screening and reimbursement criteria, modifying referrals and resources, sending referrals to appropriate navigators or case managers, and participating in daily care rounds with the multidisciplinary team to communicate changes in the patient’s condition and ensure effective transition planning. Must be capable of handling pressure, maintaining accuracy, and navigating changing situations while complying with licensing and organizational policies. BSW preferred with 2 years of medical/hospital and case management experience preferred. Starting wage is $27.31/hour with actual offered wage based on experience; the role offers non-exempt status with full-time hours in Montana.
