Inpatient Registered Nurse (RN) or Social Worker (MSW) Care Manager, Full Time, Grandview Campus (Open)

Job Summary
Develop discharge plans addressing psycho-social needs by collaborating with patients, families, and multidisciplinary teams to design safe transitions of care. Mitigate barriers to successful discharge and manage length of stay through proactive identification of issues, while re-evaluating plans as new information emerges. Coordinate utilization of community resources and address regulatory notices such as IMM and MOON to minimize readmission risk. Provide supportive counseling and advocacy to assist patients and families with illness and alternative care placement, facilitating decision-making in complex cases. Act as a resource for staff regarding regulatory issues and document all care manager functions using electronic systems.
Required Qualifications
- MSW or equivalent from an accredited school of social work
- Current NJ SW license if working at St. Luke's Warren Campus
- Graduate of professional nursing program
- Registered Nurse with current license to practice in the State of Pennsylvania or seeking Pennsylvania licensure through reciprocity
- NJ RN licensure required for Warren Campus
- BSN required or obtained within 3 years of hire
- MSN and/or Case Management Certification
- At least two years of experience as an RN or Social Worker in an acute hospital setting
- Previous care management experience
- Strong critical thinking skills
- Knowledge of medical terminology
- Ability to communicate both verbally and in written forms
- Basic computer skills
- Employment history for the past seven (7) years
Desired Qualifications
- Advanced degree
- Membership in the National Organization of Perinatal Social Workers
- BSW's currently in school within 6 months of completing MSW
- Current resume, including all work history, education, and/or certifications and licenses
