Inpatient Registered Nurse (RN) or Social Worker (MSW) Care Manager, Full Time, Grandview Campus (Open)

Job Summary
Develop discharge plans addressing psycho-social needs by collaborating with patients, families, and multidisciplinary teams to design strategies meeting individual goals. Proactively identify barriers to successful discharge, mitigate risks of readmission, and coordinate community resources while monitoring length of stay. Re-evaluate plans as new information arises and issue applicable regulatory notices such as IMM or MOON. Provide supportive counseling and advocacy for patients and families navigating illness or alternative care placements. Document all care management functions using electronic systems and facilitate complex decision-making through team meetings.
Required Qualifications
- MSW or equivalent from an accredited school of social work
- Current NJ SW license if working at St. Luke's Warren Campus
- Graduate of professional nursing program
- Registered Nurse with current license to practice in the State of Pennsylvania or seeking Pennsylvania licensure through reciprocity
- NJ RN licensure required for Warren Campus
- BSN required or obtained within 3 years of hire
- MSN and/or Case Management Certification
- At least two years of experience as an RN or Social Worker in an acute hospital setting
- Previous care management experience
- Strong critical thinking skills
- Knowledge of medical terminology
- Ability to communicate both verbally and in written forms
- Basic computer skills
- Employment history for the past seven (7) years
- Must be capable of driving a car
- Must be able to finger and handle objects frequently
- Must be able to occasionally firmly grasp, twist, and turn objects with hands and fingers
- Must be able to lift, carry, push, and/or pull objects weighing up to 75 pounds
- Must be able to occasionally stoops, bends, squats, kneels and reach above shoulder level
- Must be able to hear as it relates to normal conversations and high and low frequencies
- Must be able to touch as related to telephone and computer keyboard
Desired Qualifications
- Advanced degree
- Membership in the National Organization of Perinatal Social Workers (if primary coverage area is OB)
- BSW currently in school within 6 months of completing MSW
