Inpatient Pharmacist- Night Lead

Job Summary
Conduct overnight inpatient pharmacy services as part of a two-pharmacist team, supporting antimicrobial stewardship, medication safety, and optimized dosing for patients across oncology, surgery, medicine, cardiology, pediatrics, and critical care. Verify and evaluate the validity and safety of orders and prescriptions, integrating activities such as order verification, pharmacokinetics, IV to PO conversions, and therapeutic interchange. Provide drug information to nurses, physicians, and medical staff while assuming essential leadership responsibilities in the absence of a designated leader. Work full-time night shifts rotating through weekends and holidays. Requires a licensed Vermont pharmacist with two years of clinical experience.
Required Qualifications
- Bachelor of Science Degree from an ACPE accredited School of Pharmacy
- Licensed Pharmacist in the State of Vermont
- Two years clinical pharmacist experience in a health system
Desired Qualifications
- Doctor of Pharmacy Degree
- ASHP accredited Pharmacy Residency
- equivalent experience
