Inpatient ED Senior Clinical Care Coordinator

Coordinate care and mental health referral services for Covenant Health emergency departments, providing interventions to prevent avoidable admissions and readmissions. Assess patient care needs, determine alternative site of care options, and initiate proactive discharge planning with multidisciplinary teams. Enter services into the database and track all EMR documentation. Collaborate with medical care teams, community agencies, and families to ensure safe discharge and recognize mental health needs. Full-time evening shift includes 12-hour rotations Sunday through Tuesday and 8-hour shifts other Wednesdays. Requires a Bachelor's Degree in Social Work or Psychology, National Provider BLS certification, and three years of experience with children and families.
