Job Summary
Deliver pharmaceutical care through patient-centered clinical services, medication information, education, and medication preparation/distribution to ensure safe, effective, and cost-efficient therapy. Exercise sound clinical judgment to contribute to and implement the patient’s plan of care based on diagnosis, maintain effective communication with patients and healthcare providers, and adhere to privacy and safety policies. May perform tasks or orient other staff as needed. Requires pharmacy licensure and relevant certifications; 1 year of experience preferred. Per Diem/Casual schedule with Day/Evening shifts; hospital-based inpatient setting.
Required Qualifications
- PHARMD-Graduate of an accredited pharmacy school
- PHARMR-Current registration or Registered Pharmacist within 120 days
- ACLS-Advanced Cardiac Life Support if required by the entity/department
- 1 year of recent relevant experience
- Knowledge of NABP, TJC, Title XXII, DHS, DEA, FDA, USP
- Strong written and verbal communication skills
- Ability to work independently and as part of a team
- Familiarity with EHR systems (EPIC) and Microsoft Office Suite
