Inpatient Case Manager, RN

Inpatient Case Manager, RN coordinates comprehensive care management for hospitalized patients, focusing on transitioning patients to appropriate levels of care while ensuring clinical appropriateness and appropriate length of stay. Directly supports in-hospital patients identified for case management, promotes effective utilization and monitoring of health services, collaborates with the healthcare team and patient/caregiver to manage care and transitions, and develops and implements a comprehensive care plan addressing physical, mental, and psychosocial needs of the patient and caregiver. Works within hospital settings to improve care outcomes and transitions, with responsibilities rooted in assessment, care planning, communication, and coordination across care continuum.
