Job Summary
Inpatient Case Manager MSW responsible for delivering medical social work services in a hospital setting for Family Maternity/Women's Health, assessing social, psychological, cultural, environmental, and financial factors, collaborating with patients, families, and healthcare teams, and developing/ evaluating effective transition plans considering diagnosis, prognosis, treatment, services, and benefits to support continuum of care.
Required Qualifications
- Master Degree in Social Work (MSW)
- 1 year of acute care and/or healthcare experience (Education internship experience in an acute care facility can be considered)
- Certification in Case Management (CCM, CMAC or ACM-SW) required within one year of eligibility
- BLS required within 90 days of hire
- Prior acute care case management with Women's Health preferred
