Job Summary
Inpatient Case Manager RN responsible for comprehensive care management across the healthcare continuum with primary focus on patient transition to appropriate level of care, promoting clinical appropriateness and length of stay; collaborates with healthcare team and patient/caregiver to manage care and transitions; develops and implements comprehensive care plans addressing physical, mental, and psychosocial needs of inpatients and caregivers; functions in hospital setting with emphasis on discharge planning and coordination of services.
Required Qualifications
- Case Management certification ACM
- CCM
- CCCTM
- CMAC
- CMGT-BC
- 1 year acute care case management experience
- BSN preferred
- BLS within 90 days of hire
- RN license - Compact/Multi-State OR RN License - Single State – Virginia
- RN-Associate's Degree OR RN-Bachelor's Degree OR RN-Diploma OR RN-Master's Degree OR RN-Doctorate Degree
- Education RN-Associate's Degree OR RN-Bachelor's Degree OR RN-Diploma OR RN-Master's Degree OR RN-Doctorate Degree
