Inpatient Care Manager (RN)

Inpatient Care Manager (RN) coordinates patient care across the continuum of illness, leads the interdisciplinary team, conducts discharge planning, ensures comprehensive clinical documentation in the electronic medical record, participates in interdisciplinary rounds, monitors quality of care, and performs utilization and outcome analysis to optimize patient care and resource use. Role involves collaboration with the Supervisor/Manager of Case Management to assess, plan, implement, coordinate, monitor and evaluate services to maximize patient health, with responsibilities including chart reviews, discharge planning, and representing the department for hospital outreach.
