Job Summary
Inpatient Care Manager (MSW) coordinates comprehensive care for an identified patient population across the continuum of care, conducts assessments at entry, develops individualized care and transition plans, oversees safe/discharge planning, collaborates with interdisciplinary teams, advocates for patients/families, and maintains knowledge of Medicare/Medicaid benefits and reimbursement. Requires a Master’s in Social Work and leadership in care coordination with familiarity with EHRs (EPIC) and professional certifications (ACM/CCM). The role emphasizes discharge planning, post-acute coordination, and efforts to optimize patient care and system efficiency within LVHN. Primary site near Amity, PA with on-site responsibilities.
Required Qualifications
- Master’s Degree in Social Work
- Less than 1 year experience In an MSW internship program
- Ability to incorporate strategies for interacting with persons from diverse backgrounds
- Ability to set priorities to coordinate care plans efficiently
- Knowledge of computer applications and analytical tools
- Proven leadership skills
- Preferred Qualifications: EPIC experience
- ACM - Accredited Case Manager within 3 years or CCM - Certified Case Manager within 3 years
