Job Summary
Assess and plan care for at-risk patients in an acute care environment, coordinating services and managing discharge planning. Document interventions while ensuring regulatory compliance and advocating for vulnerable individuals. Identify barriers to care access, including social, financial, cognitive, behavioral, or legal challenges, to ensure continuity of treatment. Full-time day shift role with 40 hours weekly.
Required Qualifications
- Master's Degree in Social Work from an Accredited Council on Social Work Education program
- Texas Clinical Social Worker License - Texas State Board of Social Worker Examiners upon hire
- 2 years of social work experience in any healthcare setting
- 1 year of experience in an acute care setting or successful completion of TIPs program or Case Management Orientation Program
- Master's Degree in Social Work from an accredited CSWE program
- Texas Masters Social Worker License - Texas State Board of Social Worker Examiners upon hire
- 1 year of social work experience in any healthcare setting
- 1 year of experience in an acute care setting or successful completion of TIPs program or Case Management Orientation Program
Desired Qualifications
- Basic understanding of acute medical surgical healthcare terminology and prior case management experience
