Inpatient Care Manager

Conduct comprehensive assessments, develop and manage individualized care plans, and collaborate with physicians, nurses, and social workers to support safe, timely discharges and high-quality outcomes. Ensure regulatory compliance, accurate EHR documentation, and contribute to performance improvement. Coordinate and optimize care for hospitalized patients within the Healthcare & Medical Services department at Lehigh Valley Health Network.
