Job Summary
Innovations Waiver Care Manager coordinates complex care for members with Intellectual/Developmental Disabilities, often with secondary mental health or substance use needs, ensuring waiver requirements are met through education, discovery/assessment, and integrated care planning. Responsibilities include using person-centered planning, informing members and legally responsible persons about Medicaid/state-funded services, assisting in selecting service providers, coordinating with multidisciplinary teams, and maintaining adherence to waiver specifications for children, adolescents, and adults. The role supports remote work options with standard daytime hours and requires licensed professionals or those with relevant qualifications, plus valid NC driver’s license and NC residence within 75 miles of specified counties.
Required Qualifications
- Fully licensed by the North Carolina governing board regulating Human Services professions; OR RN licensed in NC by the NC Board of Nursing with four (4) years of full-time MH/DD/SA experience
- Master’s degree in a human service field with one year of MH/DD/SA experience (pre or post-graduate) or a substance abuse professional with one year of supervised experience in alcoholism/drug abuse counseling
- Bachelor’s degree in a human service field with two years of MH/DD/SA experience (pre or post-bachelor) or a substance abuse professional with two years of supervised experience in alcoholism/drug abuse counseling
- Bachelor’s degree in a field other than human services with four years of MH/DD/SA experience or a substance abuse professional with four years of supervised experience in alcoholism/drug abuse counseling
- Must have a valid driver’s license
- Must reside in North Carolina within 75 miles of the listed Trillium counties (Duplin, Sampson, Wayne, Lenoir) and be able to travel within catchment as required
