Job Summary
Innovation Coordinator who supports people with disabilities to secure nontraditional services combining staff supports and enabling technologies (residential, employment, and community supports) through person-centered planning, MAP's planning, and advocacy. Responsibilities include providing technical assistance to community providers, advocating on behalf of individuals/guardians, and conducting follow-ups to ensure desired outcomes, while adhering to Tennessee DDA standards. Requires a bachelor’s degree in a human services field (or equivalent experience) and ability to pass a background check, with a valid driver’s license and the ability to lift up to 25 lbs. Role is full-time and serves Montgomery County and surrounding counties; remote work option available.
Required Qualifications
- Bachelor’s degree in a human services field; or Bachelor’s in non-related field with one year relevant experience; or Associate degree with 2 years relevant experience; or High School Diploma or GED with 4 years relevant case management experience and ability to work with persons with disabilities
