Job Summary
Initiate, develop, and implement intermediate nursing plans of care for patients in the home or alternate infusion suite, evaluating progress toward goals and modifying treatments based on changing patient status or physician orders. Organize direct patient care by administering medications, performing treatments, and educating patients and families to achieve therapy independence. Assess patient needs through health data access and interviews, re-evaluate status via physical reassessment, and obtain clarifications for physician orders while providing written summaries of significant condition changes. Provide training and mentorship to Infusion Nurse I's, coordinate discharge planning, and prepare discharge summaries with patient instructions. Communicate effectively with the clinical team, medical providers, and patients to ensure continuity of care, prepare real-time clinical documentation for billing optimization, and respond promptly to patient requests or initiate emergency procedures as necessary. Participate in multidisciplinary team conferences, on-call responsibilities, and required training modules to maintain competency.
Required Qualifications
- Active and unrestricted RN license in the state of practice
- Minimum of 1 years of nursing experience
- Current CPR certification
- Required licensure to operate a motor vehicle in the state of practice
- Access to a vehicle for business travel
- Proof of liability insurance
- Basic knowledge of computer operating systems and software applications
- Ability to self-evaluate for annual appraisal
- Ability to jointly set professional growth goals with nurse manager
- Ability to apply knowledge in the effective use of nursing technology tools
- Ability to see up close and from a distance

Desired Qualifications
- Bachelor of Science in Nursing as granted by an accredited school of nursing
- 1-2 years of previous infusion nursing experience
