Job Summary
Licensed pharmacist responsible for the safe and appropriate dissemination of infusion pharmacy services, including sterile IV admixtures, medication orders processing, order assembly, and inventory while ensuring compliance with USP 797/800 and ACHC/other regulatory standards. Acts as subject-matter expert on pharmacotherapy, collaborates with patients, caregivers, and healthcare professionals, monitors lab values and pharmacokinetics to optimize regimens, participates in quality improvement and documentation, and maintains ongoing education and regulatory compliance. On-call responsibilities and active participation in the pharmacy’s programs and committees are expected.
Required Qualifications
- Graduate of an accredited School of Pharmacy
- Current, active pharmacist licensure (Pharm.D., Rph) in state of practice
- Demonstrates sufficient knowledge of sterile IV admixture and is well versed in USP 797 and 800 guidelines
- 1-2 years of clinical pharmacy experience in home care, hospital, or other infusion setting
