Required Qualifications
- Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy
- Must be licensed as a pharmacist in the Commonwealth of Kentucky
- A minimum of one year of IV infusion experience in hospital or home infusion pharmacy
