Job Summary
Home Infusion Therapy RN provides specialized infusion services in patients' homes, including IV initiation, monitoring, dressing changes, blood draws, and pump programming. Responsibilities include assessing patients, developing individualized infusion plans, educating patients and caregivers on infusion therapy and line care, collaborating with providers/pharmacists/interdisciplinary teams, monitoring for reactions and clinical changes, maintaining accurate documentation, supporting quality-improvement initiatives, and assisting with training of paraprofessional and nursing staff. Travel to patient homes and community settings is required; on-call hours may be involved. Strong communication, organizational, and critical thinking skills are essential in a flexible, mission-driven environment.
Required Qualifications
- Current New York State Registered Nurse (RN) license
- Graduate of an accredited nursing program
- At least 1 year experience with infusion therapy (IV Therapy, PICC Lines, Central Line Care, Biologics, TPN, IV Antibiotics, and/or Hydration Therapy)
