Job Summary
Provide direct patient care for assigned infusion therapy patients in home or alternate site settings, including assessments, care planning, and education on medication administration. Evaluate home environments for safety and coordinate all communications with the care team, pharmacists, and prescribers. Develop and monitor plans of care, document encounters and clinical documents per policy, and submit required treatment plans. Manage schedules via company platforms and participate in on-call rotations for continuous support. Maintain knowledge of IV access devices and infusion protocols while ensuring timely submission of all clinical paperwork.
Required Qualifications
- Must live within 25 minutes from Findlay, OH
- Must be able to participate in an on-call rotation to include 1 week every 2-3 months
- Must be able to drive your own properly registered/insured vehicle up to 1.5 hours one direction to see patients as needed
- Must be able to work in patient homes around animals/cigarette smoke
- Must have a single state RN license in good standing for the state of Ohio
- Must have knowledge of and have used IV lines, accessing ports, and administration of infusion medications
- Current RN license by the applicable state's Board of Nursing, in good standing
- Single State RN License for Ohio
- Valid state issued driver's license with no significant restrictions on that license
- Minimum 3 years nursing experience
- Must be available to respond promptly to emergencies, ensuring rapid and effective resolution of issues to minimize impact on operations
- Availability during non-standard hours, including nights, weekends, and holidays, as required by the nature of the emergency
Desired Qualifications
- RN degree, bachelor's degree in nursing (BSN) from an accredited institution preferred
- Advanced certification in specialty areas of practice (e.g., CRNI, ONS, IgRN) preferred but not required
- Experience in ICU and/or Cardiac Care preferred
- Proficient with Microsoft Outlook and Microsoft Office Suite
