Job Summary
Provide direct patient care for assigned infusion therapy patients in home or alternate site settings, including assessments, care planning, and education. Evaluate home environments for safety and coordinate all communications with the care team, pharmacists, and prescribers. Develop plans of care, document encounters, and submit required clinical documents per policy. Manage schedules via company platform and participate in an on-call rotation for nights, weekends, and holidays. Ensure timely submission of plans of treatment and maintain knowledge of IV access devices and infusion protocols.
Required Qualifications
- Current RN license by the applicable state's Board of Nursing, in good standing
- Valid state issued driver's license with no significant restrictions on that license
- Must be able to drive up to 1 hour to see patients with the flexibility to cover distances of up to 2 hours on an as-needed basis
- Must be able to participate in an on-call rotation
- Must be able to participate in an on-call rotation, nights and weekends included
- Must be available to respond promptly to emergencies
- Availability during non-standard hours, including nights, weekends, and holidays, as required by the nature of the emergency
- Minimum 3 years nursing experience
- Must be knowledgeable of IV access devices to include: IV lines, ports, and administration of Infusion medications
Desired Qualifications
- RN degree, bachelor's degree in nursing (BSN) from an accredited institution preferred
- Advanced certification in specialty areas of practice (e.g., CRNI, ONS, IgRN) preferred but not required
- Experience in ICU and/or Cardiac Care preferred
- Proficient with Microsoft Outlook and Microsoft Office Suite
- Minimum 3 years nursing experience with background in infusion therapy preferred
