Job Summary
Conduct primary patient care for assigned infusion therapy patients in home or alternate site settings, including assessments, care planning, and education. Evaluate home environments for safety and coordinate patient care via approved platforms while managing schedules and on-call rotations. Document all encounters, communications, and care activities per policy and regulatory requirements. Submit plans of treatment and required clinical documents to the branch office within acceptable timeframes. Maintain knowledge of IV access devices, infusion medications, and clinical standards. Participate in on-call rotations for nights, weekends, and holidays to address urgent issues. Ensure compliance with Soleo policies and best practices while collaborating with pharmacists, prescribers, and care partners.
Required Qualifications
- Current RN license by the applicable state's Board of Nursing, in good standing
- RN degree, bachelor's degree in nursing (BSN) from an accredited institution
- Valid state issued driver's license with no significant restrictions on that license
- Must be able to participate in an on-call rotation
- Must be able to drive up to 1 hour to see patients with the flexibility to cover distances of up to 2 hours on an as-needed basis
- Must be available to respond promptly to emergencies, ensuring rapid and effective resolution of issues to minimize impact on operations
- Availability during non-standard hours, including nights, weekends, and holidays, as required by the nature of the emergency
Desired Qualifications
- Advanced certification in specialty areas of practice (e.g., CRNI, ONS, IgRN)
- Minimum 3 years nursing experience with background in infusion therapy
- Experience in ICU and/or Cardiac Care
- Proficient with Microsoft Outlook and Microsoft Office Suite
