Infusion Nurse

Infusion Nurse role provides direct patient care for infusion therapy in home/alternate site settings. Responsibilities include initiating and maintaining communication with care team, educating patients and care partners on safely administering home infusion therapy, evaluating home environment for safety, developing and evaluating plans of care with the team, managing the schedule, coordinating patient care via company platform, participating in on-call rotation, documenting encounters per policy, maintaining knowledge of Soleo therapies and clinical standards, ensuring timely submission of POTs and clinical documents, and ensuring required patient documents are completed and returned to the branch.
