Infusion Nurse

The Infusion Nurse provides direct patient care for infusion therapy in the home or alternate site, including assessments, care planning, patient/care partner education, and coordination with the care team. Responsibilities include initiating/maintaining communications with pharmacists, prescribers, and care partners; evaluating home environments for safety; developing and evaluating plan of care with the team; managing scheduling via company platform; participating in an on-call rotation; documenting all patient encounters per policy; maintaining knowledge of Soleo therapies and protocols; ensuring timely submission of POTs and related documents; and handling administrative oversight and emergencies as required. Requires RN licensure in GA, a BSN is preferred, and 3+ years infusion/ICU or cardiac care experience is preferred. Must be able to drive up to 2 hours occasionally and participate in on-call rotations; on-site home infusion settings are expected.
