Infusion Nurse II

Initiate, develop, and implement intermediate nursing plans of care for patients in home or alternate infusion suites, evaluating progress toward established goals. Administer medications, perform treatments, and educate patients and families on prescribed therapy to achieve independence. Modify treatment plans based on changing patient status or physician orders, while assessing needs through health data access and physical reassessment. Provide training and mentorship to Infusion Nurse I's, coordinate discharge planning, and prepare discharge summaries with patient instructions. Communicate effectively with the clinical team, medical providers, and patients to ensure continuity of care, and prepare clinical documentation in real-time for billing optimization. Respond promptly to patient requests, initiate emergency procedures as necessary, and act as supervisor in the absence of the Nurse Manager. Participate in multidisciplinary team conferences and complete required training modules to maintain competency.
