Job Summary
Collaborate with referral sources, account managers, and clinical teams to triage new cases and present issues impacting initial case assignments. Respond to outpatient referral inquiries, gather demographic data, collect preliminary client information, and assist in scheduling initial appointments. Manage acute and chronic therapies including IVIg, TPNs, and antibiotics while ensuring all patient information is recorded correctly. Work closely with field healthcare professionals to ensure seamless care delivery and confirm patients receive proper care. This role supports the enrollment process for Always Compassionate Health, a leading New York home health provider serving thousands of clients daily.
Required Qualifications
- Excellent organizational and time-management skills
- At least 3-5 years of experience in a Home Care setting
- Strong communication and interpersonal skills
- Attention to detail with problem solving skills
- Strong customer service background
- Ability to work in a fast-paced environment and demonstrate ability to multi-task
