Job Summary
Evaluate user health status at the CHSLD, plan and deliver nursing care, and ensure follow-up. Collaborate with the interdisciplinary team to maintain care continuity and provide education to users and their families. Maintain complete and compliant clinical documentation. Requires recent CHSLD experience, DCI (Crystal Net), and SIL-P training. Full-time day shift rotation from Wednesday to Tuesday, 6:45 AM to 3:00 PM, with 45-minute meal break. Valid driver's license and vehicle required. OIIQ membership mandatory.
