Job Summary
Conduct comprehensive physical examinations, prescribe treatments, and develop patient care policies for geriatric residents in skilled nursing facilities. Coordinate advanced care planning, discharge visits, and referrals to specialists while ensuring timely documentation of assessments and treatment plans. Meet a minimum of 12 patient visits daily, with bonuses for exceeding targets. Provide clinical leadership by collaborating with physicians and the interdisciplinary team to prevent re-hospitalizations and address gaps in care. No nights or weekends; flexible full-time schedule.
Required Qualifications
- Active Florida Nurse Practitioner License (unencumbered)
- Master's Degree in Nursing from an accredited program
- Strong clinical background in Infectious disease
- Ability to engage and communicate effectively with elderly and chronically ill patients and their families
- Ability to meet a minimum of 12 patient visits per day
Desired Qualifications
- Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long-term care, hospice, palliative care, or similar settings
- Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus
