Job Summary
Infant Case Manager at Sea Mar focuses on improving client self-sufficiency in accessing and coordinating care for infants and families. Responsibilities include six core services: health promotion, client/family support, care coordination, referrals to social and community resources, and development of care plans with measurable goals. The role emphasizes advocacy, education, and empowerment to utilize preventative health and social services, with regular caseload management, interdisciplinary team collaboration, and the use of motivational interviewing and behavioral activation techniques. Requires working with parenting low-income families meeting high-risk criteria, with travel to Western Washington and a workload expectation of approximately 20 visits per week. Bilingual English/Spanish preferred; comprehensive benefits provided and annual influenza vaccine required.
Required Qualifications
- Bachelor’s or Master’s degree in a social service related field such as social work, behavioral sciences, psychology, child development; ability to work with underserved populations; experience with community social services; ability to coordinate with behavioral health providers; proficiency in using electronic health records; BLS CPR within 90 days; Washington State Patrol background check; influenza vaccine requirement.
- One year of experience in community social services, public health services, crisis intervention, outreach and referral programs or related field.
- Ability to travel to Western Washington as needed.
