Job Summary
Conduct weekly contact and home visits with residents to co-create goals and develop steps forward across various life areas. Provide individual support and structure while collaborating with support guides, house coaches, and external partners like family and treatment providers. Manage medication administration for residents unable to self-manage during weekend rotations and maintain group dynamics on housing sites. Work within the Krachtwerk methodology to focus on residents' capabilities while navigating psychiatric issues, addiction, and homelessness.
Required Qualifications
- Een hbo- of wo-diploma
- Kennis van en inzicht in de OGGZ, psychiatrische problematiek en verslaving
- Ken je de regels van de Wmo en de Wlz
- Ervaring met onze doelgroep: werken met mensen met psychiatrische klachten, sociaal kwetsbare mensen en met dak- en thuisloze mensen
- Een jaarcontract voor 24-32 uur per week
- Fulltime dienstverband
Desired Qualifications
- Bij voorkeur in de richting Social Work, SPH, SJD of Psychologie
- Een andere opleiding in de zorg
- Zelf ervaring hebt met psychische klachten of verslaving
- Een opleiding tot ervaringsdeskundige hebt gedaan
