Individual Supports Program Coordinator

Job Summary
The Individual Supports Program Coordinator role involves advocating for individuals receiving services and ensuring their rights and dignity are respected. Responsibilities include providing direct supports and case management for individuals, developing and monitoring Individual Support Plans (ISPs), and maintaining communication with families, service coordinators, and health providers. The role requires managing documentation, participating in a rotating on-call schedule, and supporting individuals in accessing community resources. Candidates should have experience with developmental disabilities, strong organizational and communication skills, and possess an Associate’s Degree in a related field.
Required Qualifications
- At least two (2) years of experience supporting individuals with developmental disabilities
- At least one (1) year of supervisory experience
- Strong organizational skills
- Computer competency
- Reliable personal vehicle and valid driver’s license
- Ability to lift up to 30 pounds
- Strong written and verbal communication skills
- Ability to exercise sound judgment and work independently
- Bilingual Spanish/English strongly preferred
Desired Qualifications
- Bachelor’s Degree preferred
