Job Summary
Indigenous Patient Navigator role within the Indigenous Health program focused on supporting FNIM patients and families to access culturally safe care. Responsibilities include receiving referrals, navigating the pediatric healthcare system, facilitating access to translation and traditional wholistic practices, coordinating non-insured benefits and transportation, promoting culturally safe care, supporting care transitions, and collaborating with interdisciplinary teams. The position emphasizes advocacy, program development for NEO Kids & Family, and the use of culturally informed practices to improve health outcomes for Indigenous children and families. Requires a post-secondary health sciences/social sciences diploma, professional regulatory registration, safety training, 3+ years of Indigenous health experience, and strong communication, organizational, and data-management skills.
Required Qualifications
- Minimum of a post-secondary Diploma in Health Sciences, Social Sciences, or Indigenous specialization in a related field, from an accredited community college
- Current Certificate of Registration with a regulatory college of the applicable professional discipline
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required
- Minimum of three (3) years’ experience working within an Indigenous health care or social services environment
- Experience working with FNIM people’s language, culture, customs, and traditions is required
- Experience working with children and their families and community is required
