Job Summary
Assist individuals in 1:1 in-home and in-community settings by supporting daily activities like personal care, meal prep, transportation, housekeeping, and laundry while fostering independence. Provide skill development in community participation, health, safety, household management, and adaptive skills, including accompanying individuals on errands, appointments, and social events. Maintain clean, safe spaces and attend team meetings to write progress reports and ensure consistency in service provision. Drive a personal vehicle within a 30-mile radius to transport person-served, with mileage reimbursement offered. Work full-time between 9am and 7pm, with schedules subject to change based on individual needs.
Required Qualifications
- Valid driver's license with acceptable driving record, auto liability insurance, and a reliable vehicle to transport person-served
- Must be at least 18 years old or 21 years old depending on the person-served
- High school diploma or GED
- Associates in relevant field OR two years equivalent experience in working with developmentally disabled and/or mental illness, as determined by the supervisor
- Must be responsible, reliable, and able to carry out job functions without direct supervision
- Willing to drive a 30-mile radius
- Adept in the use of technology and possess a smart device with adequate data
- Must be flexible and a team player, willing to provide the resources and positive support needed for the person-served
- Good communication skills to document care, follow support plans, and collaborate with both internal and external team members
