Job Summary
Assist individuals in 1:1 in-home and in-community settings by supporting daily activities like personal care, meal prep, transportation, housekeeping, and laundry. Foster independence through skill development in community participation, health, safety, household management, and adaptive skills, including accompanying person-served on errands, appointments, and social events. Maintain clean, safe spaces while attending team meetings, writing progress reports, and collaborating with support team members. Provide coaching and mentoring to help individuals reach their personal goals. Full and part-time caseloads available between 9am and 7pm. Requires a valid driver's license, reliable vehicle, and willingness to drive within a 30-mile radius. Must be at least 18 years old (or 21 depending on person-served) with a high school diploma or GED.
Required Qualifications
- Valid driver's license with acceptable driving record, auto liability insurance, and a reliable vehicle to transport person-served
- Must be at least 18 years old or 21 years old depending on the person-served
- High school diploma or GED
- Associates in relevant field OR two years equivalent experience in working with developmentally disabled and/or mental illness, as determined by the supervisor
- Must be flexible and a team player, willing to provide the resources and positive support needed for the person-served
- Good communication skills to document care, follow support plans, and collaborate with both internal and external team members
- Adept in the use of technology and possess a smart device with adequate data
- Willing to drive a 30-mile radius
