Job Summary

Independent Living Specialist responsible for all aspects of assigned caseload in a Harlem transitional housing program, including meeting with clients at least weekly, making referrals to medical, mental health and substance abuse services, providing supportive counseling, conducting regular client visits to ensure safety, teaching and assisting with activities of daily living, completing housing applications and psychosocial reports, providing appropriate level of support including escorts when needed, maintaining updated clinical files, participating in intake and admission processes, and engaging in community meetings. Role involves documentation and coordination to support residents in rehabilitation and achieving work, social, and community living goals; bilingual Spanish required, CPR training or willingness to obtain, and HS diploma/ GED with significant relevant experience. Required Qualifications
