Independent Living Specialist responsible for all aspects of assigned case load, including meeting with clients regularly, ensuring clinical files and reports are complete, assisting residents in obtaining documentation, escorting to appointments, securing entitlements, preparing housing packets (HRA2010E), arranging housing interviews with providers, building clients’ daily living skills, facilitating groups under the Program Supervisor, and liaising with clinical staff across programs to achieve successful outcomes through the continuum of care.
