Independent Living Specialist - 2929 Third Ave

Independent Living Specialist responsible for a caseload of formerly homeless individuals with mental health and/or substance use disorders. Duties include intake/admission, eligibility and entitlements processing, development and updating of service plans, maintaining chart documentation, providing supportive counseling, facilitating support groups, teaching activities of daily living, assisting with money management and rent collection, arranging recreational activities, and making linkages to medical, mental health, and substance abuse resources. Employ harm-reduction approaches and motivational interviewing, conduct regular home visits to residents, participate in interdisciplinary and community meetings, be on call after hours, provide emergency first aid/CPR when needed, and report to the Program Supervisor. Based in Bronx, NY with full-time schedule.
