Job Summary
Independent Living Specialist responsible for comprehensive case management of identified chronic homeless individuals. Manage full caseload with field and office meetings, maintain clinical files and reports, conduct comprehensive assessments to identify housing barriers, assist clients to overcome barriers and secure entitlements, develop and implement individualized housing plans, build a network of support services, prepare clients for housing interviews and coordinate housing appointments, participate in case conferences, liaise with clinical staff, provide emergency first aid/CPR or overdose prevention as needed. Strong focus on housing stability within program continuum; requires experience in residential treatment, knowledge of HRA/NY/NY systems, and bilingual Spanish skills preferred.
Required Qualifications
- High School Diploma/GED required
- Bachelor's degree preferred
- Masters degree ideal
- CPR training certification or willingness to take training class in CPR
- Bilingual Spanish strongly preferred, but not required
- Excellent communication and writing skills
- Computer proficiency
- Experience working in residential treatment and with the MICA and homeless populations
- Knowledge of HRA and NY/NY application systems a plus
