Job Summary

Impart agency philosophy of independence to clients, families, and community resources while maintaining strict confidentiality. Provide training to program participants to help them acquire skills for more independent living situations and conduct functional assessments of daily life activities. Work collaboratively with Service Coordinators to advocate for consumers, access safe housing, and develop comprehensive service plans. Accompany participants to community locations to teach resource utilization and assist with recreational and social activities. Act as a role model to develop behaviors necessary for successful community living and implement behavior modification plans as needed. Submit monthly reports and promote communication with the team regarding treatment plans and safety issues.

Required Qualifications

  • Knowledge of community resources and means of accessing services
  • NYS driver's license
  • Licensed Master Social Worker (licensed by the New York State Education Department pursuant to Article 154 of the New York State Education Law)
  • Licensed Clinical Social Worker (licensed by the New York State Education Department pursuant to Article 154 of the New York State Education Law)
  • Individual with a Doctorate or Master of Social Work
  • Individual with a Doctorate or Master of Psychology
  • Licensed Physical Therapist (licensed by the New York State Education Department pursuant to Article 136 of the New York State Education Law)
  • Registered Professional Nurse (registered by the New York State Education Department pursuant to Article 139 of the New York State Education Law)
  • Certified Teacher of Students with Disabilities (certified by the New York State Education Department)
  • Certified Rehabilitation Counselor (certified by the Commission on Rehabilitation Counselor Certification)
  • Licensed Speech Pathologist (licensed by the New York State Education Department pursuant to Article 159 of the New York State Education Law)
  • Licensed Occupational Therapist (registered by the New York State Education Department pursuant to Article 156 of the New York State Education Law)
  • Individual with a Master of Gerontology
  • One (1) year of experience completing functional based assessments of Activities of Daily Living and Instrumental Activities of Daily Living, developing comprehensive treatment plans, and teaching individuals with Traumatic Brain Injury to be more functionally independent
  • Bachelor's degree and three (3) years of experience completing functional based assessments of Activities of Daily Living and Instrumental Activities of Daily Living, developing comprehensive treatment plans, and teaching individuals with Traumatic Brain Injury to be more functionally independent
  • Associate's degree and five (5) years of experience completing functional based assessments of Activities of Daily Living and Instrumental Activities of Daily Living, developing a comprehensive treatment plan, and teaching individuals with Traumatic Brain Injury to be more functionally independent

Desired Qualifications

  • Experience working with individuals with traumatic brain injury providing functionally based assessments and independent living skills training
