Job Summary
Coach and teach independent living skills based on each person's unique goals and strengths while partnering with individuals to develop personalized, person-centered plans. Support healthy living through nutrition, wellness, exercise, and daily routines, and encourage participation in community activities, recreation, volunteering, and relationship building. Assist individuals in exploring employment interests and developing workplace readiness skills, then collaborate with families, support teams, and community partners to provide coordinated services. Track progress, document outcomes, and celebrate achievements along the way. This role requires a valid California driver's license, negative TB test, occupational physical clearance, and DOJ/FBI fingerprint clearance. Full-time position in San Jose with $24–$25/hour compensation and benefits including medical, dental, vision, and 401(k) match.
Required Qualifications
- Valid California Driver's License (or California ID where applicable)
- Negative TB test
- Occupational physical clearance
- DOJ/FBI fingerprint clearance
- Must be at least 18 years old (or possess appropriate work authorization if applicable)
- Must be able to Drive and transport adults served safely throughout the community
- Must be able to Participate in community, recreational, vocational, and household activities
- Must be able to Lift up to 25 pounds as needed
- Must be able to Perform physical activities throughout the workday
Desired Qualifications
- One or more years of work or volunteer experience supporting adults with disabilities
- Experience working in professional or community-based settings
- Associate degree (completed or in progress)
