Job Summary
Provide comprehensive case management for youth in the Independent Living Program, coordinating services and collaborating with foster parents/caregivers and service-provider staff to achieve Independent Living goals for youth aged 13–21. Responsibilities include managing dependent children’s case plans, exploring permanency options, staying current with IL-related laws, attending IL Transition and Education Plan staffings, coordinating transport for staffing, incorporating IL assessments into transition plans, completing staffing paperwork and required packets, performing monthly home visits, helping youth obtain a Florida ID, open bank accounts, explore housing needs, ensuring youth 16–17 receive credit checks, guiding youth through transition topics (money management, employment, education, relationships, activities, driver’s education), serving as liaison between teens and foster caregivers, tracking progress in IL using the automated system, and identifying community resources to support training and independent living needs, while respe...
Required Qualifications
- Bachelor’s Degree in Social Work or related field
- Certified Child Welfare Professional
- Valid Florida Driver’s License
- Two (2) years of experience in Child Case Management
