Job Summary
Conduct in-home health evaluations across New York and neighboring states, including medication reviews, physical exams, and diagnostic tests like spirometry and diabetic eye exams. Perform virtual visits as needed while managing regions within licensed states, requiring flexibility for up to 100% travel with overnight stays and car rentals. Utilize an iPad clinical workflow for each 45-minute visit to connect one-on-one with patients in their private residences. This role supports Signify Health's mission to bridge care gaps by providing personalized, comprehensive health services directly to communities nationwide.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Able to cover all counties within New York
- Flex to other areas of business need
- Licensed in one or more of these states: NY, ME, MA, RI, or CT
- Manage regions across your licensed states
- Travel is a key component—including car, train, and air travel—with overnight stays required for visiting various markets
- Be flexible to travel locally and within licensed states, up to 100% of the time
- Able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
