Job Summary
Conduct 45-minute in-home health evaluations across Minnesota, performing medication reviews, physical exams, and diagnostic tests like spirometry and diabetic eye exams using an iPad clinical workflow. Travel up to 100% of the time within licensed states, including overnight stays and flights, to visit millions of patients nationwide. Perform all Signify Health services including preventive screenings while maintaining relationships with health plan members in diverse home environments. Cover all counties within Minnesota with multi-state licenses.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Able to cover all counties within Minnesota and flex to other areas of business need
- Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
