Job Summary
Conduct in-home health evaluations across a defined coverage area, performing 45-minute visits that include medication reviews, physical assessments, and diagnostic tests like spirometry or diabetic eye exams. Utilize an iPad and clinical workflow for each evaluation while engaging patients one-on-one to bridge care gaps. Travel locally and within licensed states up to 100% of the time, managing overnight stays and car rentals as needed. Maintain active, unrestricted multi-state medical licenses and adapt to diverse home environments that may include pets or allergens. Work full-time hours with comprehensive benefits including malpractice coverage and paid time off.

- Active, unrestricted license(s) in coverage area(s)
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Holding multiple state licenses
- Must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
