Job Summary
In-Home Traveler Nurse Practitioner or Physician Assistant needed to provide in-home health evaluations across a defined coverage area. Travel up to 100% of the time with potential overnight stays; use iPad for clinical workflow; visits include medical history review, physical evaluation, and optional diagnostic tests as ordered by health plan. Requires active, unrestricted licensure in coverage area(s), board certification, and ability to obtain/maintain multi-state licenses. Anticipated weekly hours: 40. Full-time clinician will join Signify Health within CVS Health network and may travel locally and across licensed states; in-home primary workplace communities and flexible benefits available.
