In-Home Traveler Nurse Practitioner or Physician Assistant (Full Time) traveling clinician to perform in-home health evaluations for Signify Health, part of CVS Health. Conduct 45-minute visits including medical history review, physical examination, and, if ordered by the health plan, additional diagnostic tests (e.g., spirometry, bone density screening). Use an iPad and clinical workflow; no prescribing, testing, or treatment changes required during visits. Requires active NP or PA license with board certification and ability to obtain multi-state licenses; travel up to 100% of the time across a defined coverage area with potential overnight stays. Anticipated weekly hours: 40. Salary range: $87,035.00–$187,460.00; benefits include health/dental/vision, 401(k), PTO, CEU allowance, malpractice coverage.
